Mechanism of Action of Codeine Phosphate
Although the specific mechanism of action of codeine is uncertain, it is widely assumed to be mediated via opioid receptor antagonism, particularly mu-opioid receptors. Morphine was originally thought to contribute to codeine’s analgesic action through O-demethylation by CYP2D6. Codeine’s analgesic impact was particularly absent in CYP2D6-poor metabolizers. Only 5% of codeine is converted to morphine, therefore this is unlikely to be the primary mechanism of action. Other hypotheses propose that codeine-6-glucuronide, codeine’s primary metabolite, mediates the analgesic effect of codeine because it has the same affinity for mu receptors as codeine but can also be converted to morphine-6-glucuronide.
